APM Communities

APM has been selected to deliver National Disability Insurance Scheme (NDIS) Scheme in selected areas of Western Australia. The Local Area Coordinators (LACs) help people with a disability, NDIS, families & carers to identify and access the support they need. This includes helping them to realise their individual goals and aspirations by connecting them to the community & maintain services. LACs also work with these services & organisations to more inclusive of people with a disability.

Ark Support Coordination

Ark Support Coordination is an independent, individualised, family-focused service that only offers support coordination for NDIS participants. Being independent means their organisation works purely for the person, with no conflict of interest to offer other services within the same organisation. Several Ark employees have children with ASD, and their value of lived experience is a significant driving force for their practice. As of August 2020, social worker, support and specialist support coordinator Tain Molendijk will be bringing Ark’s services to the south west. specifically to the Augusta-Margaret River region.
