Information, Linkages & Capacity Building

SWAN have received a grant from Department of Communities - Disability Services to deliver an Information, Linkages and Capacity Building (ILC) Project from 1st July 2018 to 30th September 2019. Our project is to connect people with disability to mainstream services in the south west region of WA.

Please let us know what you need to better access mainstream services in the south west by answering our 5 minute survey.

We will be providing:

  • Information and referral to appropriate mainstream services by phone, email, Facebook and face-to-face.
  • Four print resources describing what supports are available, and answering your most frequently asked questions.
  1. Early Years (0-6yrs)
  2. Primary School (7-12yrs)
  3. High School (13-18yrs)
  4. Adulthood (18yrs +)
  • Monthly carer support group meetings in partnership with Carers WA in Busselton, and support to improve capacity of existing peer support groups in Margaret River, Bunbury and Bridgetown-Manjimup.
  • Information workshops in local South West communities.
  • Advocacy, both individual and systemic.
  • Expansion of existing Free Resource Library program.
  • A 2018/2019 mainstream and disability service providers directory.
  • Plain English and Easy Read resources developed in collaboration with Down Syndrome WA for distribution, prioritising people with Intellectual Disability, Autism, and related developmental disabilities (e.g. use of mind maps for determining individual goals of person with disability).
  • Personalised peer support.
  • Regular information updates through Facebook.
